Care home support
The Care Home Support Team supports care homes in Bristol, and in particular care homes with facilities for nursing.
Our primary role is to help improve the quality of care provided in care homes in Bristol and to support care home staff to do their job to the best of their ability. We provide training and support to implement good practice.
Our team work helps to reduce organisational safeguarding issues in care homes with nursing and reduce avoidable admissions to hospitals.
The Care Home Support Team consists of trained nurses, our Care Home Support Team managers and a team administrator.The core team are supported by a virtual team, who provide specialised support in their area of expertise which include:
- Dietetics Support
- Medicine Management Support
- Specialist Community Services inc Therapy and Bristol Community Health (BCH)
- End of Life (EOL) / Palliative Care
What support can we provide?
The core team make contact with the care home manager and begin scoping work to understand how the care home runs. This often including shadowing and talking with care home staff.
The team identify any potential areas of improvements and/or causes of concern. These are discussed with care home staff, manager and the clinical lead to decide on the most effective approach to assist the care home to make sustainable improvements.
These improvements might include training, and reviewing care home processes and procedure. Alternatively, the team may look to a member of their virtual team to help address a particular issue such medicine management, nutrition or end of life.
