Better Care Bristol
About the Better Care Fund
The Better Care Fund is a national programme to improve the ways health services and social care services work together. It creates pooled funding arrangements in local areas to support NHS organisations and the local government to jointly plan and deliver services, with focus initially on services for older people and people living with long-term conditions.
The Government announced the Better Care Fund in June 2013 and is seen as a real opportunity for change, so that people receive the right care and support, at the right time, in the right place.

Bristol Better Care Plan
Our Better Care plan will focus on all adults in Bristol and was developed by Bristol Clinical Commissioning Group (CCG) and Bristol City Council in partnership with the main health and social care providers. The plan has been agreed by the Health and Wellbeing Board and is approved by NHS England and is subject to annual refresh.
Our programme is part of an on-going process to transform local health and social care services, to improve the lives of people in our community through joint working and joint planning of services for the benefits of patients, carers and their families.
In Bristol we are working in partnership with University Hospitals Bristol, North Bristol Trust, the Avon and Wiltshire Mental Health Partnership Trust (AWP) and BrisDoc, as well as community providers including Bristol Community Health, Bristol Ageing Better and Healthwatch Bristol.
Better Care Fund Planning Submission
The Better Care Programme in Bristol will be used as a way of supporting delivery of the CCG and Council's strategic aims into action plans in relation to Health and Social Care
Our Transformation Programme focuses on the following areas:
Help you to help yourself to be well
Establish an integrated approach to wellbeing that addresses healthcare inequalities and provides individuals with the appropriate advice and guidance to make informed decisions about their own care and support, to enable them to manage their own health and social care needs more effectively.
Provide care in the right place
Ensure there is the right support available for people when they need services to support their urgent Health and Social Care needs. It’s important that when someone requires acute medical care, it’s coordinated and managed to ensure the patient spends as short a time as possible in hospital, and they are able to leave as soon as possible with appropriate support to get people home as soon they are well enough.
Support you to be independent for longer
Ensuring, where necessary, people get the on-going support they need to be safe and to live as independently as possible, either in their own homes, care homes or in extra care housing.
Better Care Fund Approval Letter
Letter from Dame Barbara Hakin, National Director for Commissioning Operations NHS England sent to Bristol CCG informing us that our plan has been classified as ‘Approved’.
